Summary:Epilepsy surgery is the most efficient therapy for patients with pharmacoresistant epilepsy. Patients with epilepsy refractory to pharmacotherapy should receive consultation about the suitability of a surgical solution. This chapter discusses the investigations and indications for epilepsy surgery. It describes the semi‐invasive and invasive video EEG. The results of electroencephalography (EEG), especially of ictal video EEG, are compared with those of imaging and metabolic methods as well as neuropsychological findings. The cornerstone of epilepsy diagnostics is video EEG recording of seizures. Epilepsy surgery can be curative or palliative. The goals of surgery are to remove the epileptogenic lesion and epileptogenic zone, to halt disease progression, to prevent the spread of seizure activity, and to inhibit seizures via neuromodulatory techniques. Other methods are being developed, including gamma knife radiosurgery and thermolesioning using inserted depth electrodes.
