Summary: | Amaç: Perkutan etanol enjeksiyonu tedavisi uygulanan otonom tiroid nodüllerihin değerlendirilmesinde renkli Doppler ultrasonografinin etkinliğinin araştırılması. Gereç ve Yöntem: Otonom tiroid nodüllü 21 olguya (15 kadın, 6 erkek, ortalama yaş 44) perkutan etanol enjeksiyonu tedavisi uygulandı. Olgular tedavi sonrasında en az bir yıl takip edildi. Olgular tedavi öncesinde, tedavi ve takip evresinde renkli Doppler ultrasonografi ile değerlendirildi. Nodüller kanlanmalarına göre Grade 0,1,2,3 olarak derecelendirildi. Doppler bulguları gri skala ultrasonografi bulguları, T3, T4, TSH ve sintigrafi bulguları ile karşılaştırıldı. Bulgular: Nodüller soliter olup hacimleri 1,8-13,0 cm3 arasındaydı. Vaskülarite bir olguda grade 1, 9 olguda grade 2, 11 olguda grade 3 bulundu. Etanol enjeksiyonu haftada 2 kez olmak üzere vasküler skorda grade 3 olgularda en az iki derece, grade 2 ve 1 olan olgularda en az bir derecelik düşüş saptanana kadar devam edildi. Ondokuz olguda (% 90,4) tedavi başarılı oldu. Sonuç: Etanol enjeksiyonu ile tedavi edilen soliter otonom tiroid nodüllerinde renkli Doppler ultrasonografi bulguları tedavinin yönlendirilmesi ve etkinliğinin araştırılmasında faydalı bir yöntem olarak görüldü.
Purpose: The evaluation of the effectiveness of color Doppler sonography in the autonomous thyroid nodules treated with percutaneous ethanol injection. Material and Method: Twenty-one patients with autonomous thyroid nodules were performed percutaneous ethanol injection. Fifteen patients were female and six patients were male. The mean age was 44. The cases were followed at least one year after the treatment. The cases were evaluated by color Doppler sonography before, during and after the treatment. Nodules are evaluated according to their vascularity as grade 0, 1, 2 and 3. Sonographic findings were compared with T4, 73, TSH levels and scintigraphic findings. Findings: All nodules in patients with autonomous thyroid nodules were solitary and their volumes were between 1.8 - 13.0 cm-3. Vascularity was grade 1 in one case, grade 2 in 9 cases and grade 3 in 11 cases. Percutaneous ethanol injection was performed twice a week until at least two grade decrease in grade 3 was achieveol and at least one grade decrease in grade 1 and 2. The treatment was successful in 19 cases (90.4 %). Result: Color Doppler sonography is the effective method in the evaluation of autonomous thyroid nodules treated with percutaneous ethanol injection. |
