Summary:The proportion of elderly people has increased in the developed world. This age group receives a great number of prescriptions. Persons of advanced age take many drugs for a number of diseases, a fact that increments the risk of adverse reactions and of interactions. The pharmacist plays an important role in patient discharge since he or she can provide written and oral advice and instructions concerning the drugs being prescribed to the patient. A study of the high-risk drugs that are prescribed at the time of hospital discharge was carried out. Many of these drugs are responsible for the major geriatric syndromes. [publication abstract]
