Summary: | Clinical risk rating tool An oft-quoted figure from the Institute of Medicine is that there are 98,000 deaths annually because of preventable errors in healthcare.6 The question within the healthcare supply chain should not be whether a product will fail, but what will be the severity, detectability and potential frequency when the product fails because of human, design or manufacturing error. (See Figure 1 and the sidebar, "Clinical Risk Rating Decision Tree," p. 31, for more details.) Since developing the tool, the clinical services team used it during the past two years to evaluate pharmaceutical, medical and surgical products being sourced for its member hospitals. A byproduct of this preoccupation with failure is the development of situational awareness, and a more dynamic and realistic evaluation of the competitive environment with better preparation for resilience in the face of potential product failure.
