Summary:The purpose of this study is to present HLA compatibility of kidney recipients and donors transplanted in our centers as well as the correlation of the percentage of one to 5 years graft survival rate according to the degree of HLA class I matching and patients senzibilization. The study has evaluated 302 first living transplantation performed from 1996 to 2003. There were 21 sibling (7 of them were identical), 265 parent, 13 other relatives and 3 unrelated donors. HLA compatibility was evaluated by serologycal typing of patients and donors. Cross matches and screening of anti-HLA antibodies in patient sera were performed by standard CDC and DTT-CDC where necessary. There were 12 class I match pairs and 44, 241 and 5 with 1, 2 and 3 mismatches respectively. Eightyseven patients had pretransplant cytotoxic antibodies out of those 35,6% with PRA greater than or equal to 50%. Percentage of one year patients survival rate was 98,7% while graft survival rate was 92,7%. Influence of Class I, locus A, locus B mismatches and %PRA on 1-5 years patients and graft survival rate was shown. In the group of patients with graft survival less than 1 year there were 36,4%, while among the others there were 27,5% senzibilized patients. A five-year graft survival was 86,1%, 75,6% and 52,4% in patients with PRA less than or equal to 10%, 15-45% and greater than or equal to 50% respectively. Obtained data confirmed the significance of HLA compatibility degree in the choice of kidney transplantation pairs. However, in our group of patients it was indicated that senzibilization had more influence on long graft survival than the Class I mismatches.
