Summary:The spine is one of the most common sites for tumor metastasis which account for a decreased quality of life and 15 mortality.This study aimed to determine the Neutrophil-to-Lymphocyte Ratio (NLR) as one of the predictors of spinal 16 metastasis.This descriptive cross-sectional study was conducted at Wahidin Sudirohusodo Hospital Makassar, South 17 Sulawesi, between July 2018 and July 2022. Data were obtained retrospectively from medical records and analyzed. 18 From a total of 300 patients, 68 (22.7%) patients had spinal metastasis,particularly thoracic vertebrae(35.3%), and 83 19 (27.7%) patients had significant NLR values. According to NLR analysis, there was a significant difference of mean 20 NLR between the tumor types and between the metastatic and non-metastatic groups (p<0.001), which significantly 21 higher in metastatic group (6.10±5.16% and 6.10±5.16%)than breast tumors (2.27±1.02%) and non-metastatic group 22 (2.67±1.61%), respectively. Based on the accuracyanalysis, there was a significant relationship between NLR cut-off 23 and metastatic status (p<0.001) with the sensitivity of 57.4%, specificity of 81.0%, positive predictive value of 47.0%, 24 negative predictive value of 86.6%, accuracy of 75.7 %, AUC of 0.728 (p<0.001) and the NLR cut-off was2.52.NLR can 25 be a useful predictor of spinal metastasis and can be applied to metastatic spinal tumors.
