African American Type 2 Diabetes Patient Post-discharge Education as a Predictor of 30-Day Hospital Readmission

In the United States, type 2 diabetes is prevalent among the African American population. This quantitative correlational study examined to what extent adult African American Type 2 Diabetes patients’ post-discharge education and close follow-up, separately and together, predicted their 30-day hospital readmissions of residents in Kent County, Delaware. A logistic regression was used to measure significance in the relationship between post-discharge education, close follow-up, and 30-day hospital readmissions. Archived data for the year 2017, were provided by the Delaware Health Information Network. A review of 184 archived claims data, resulted in a sample of 64 African Americans’ claims data for this research. The study employed the Eraker, Kirscht, and Becker’s health decision model, as the theoretical foundation to determine the correlation between patient education, and close follow-up, and patient outcomes. The results showed that patient education separately, and together with close-follow-up were untestable due to insufficient data, however, close follow-up as a single variable was statistically, insignificantly correlated with 30-day hospital readmissions. It was also revealed that a disparity existed in the manner in which data and records were collected and maintained for African Americans during the year 2017, in Kent County, Delaware.
