Summary:During phase two of the pilot, the nurse coordinator mentored the SDC through creation of SCPs and evaluated completeness and accuracy. Because the SDC was not a clinician, the cancer committee required oversight and review of the plan by a nurse and approval by a clinician prior to presenting it to a patient. Job Description The second phase of the pilot was deemed a success because the number of patients receiving SCPs increased and resulted in greater nurse, patient, and provider satisfaction. Results The complexity of implementing SCPs justified a dedicated staff member to facilitate case findings, develop systems to track patients through multifaceted treatment regimens, abstract data from the EHR, coordinate timely delivery of SCPs, and monitor the number of delivered plans. Nurses report that patients appreciate the SCPs; however, they observed that the plan might trigger anxiety in some patients because of the discussion of late-term effects and risks of secondary cancers.
