The study is focussed on those accidents that occurred during maintenance or cleaning operations, involving chemical reactivity hazards. The objective is to obtain lessons learned by studying the causes and consequences of such accidents so that similar situations can be avoided in the future. Safety management systems should incorporate the lessons learned in order to improve the overall safety of the procedures of routine operations to be performed in an establishment.
