Summary:"Unexposed bone variant" account for up 1/3 of all ONJ cases; these cases have been included in the so-called "stage 0" of the 2009 AAOMS staging system and can cause underestimation of ONJ frequency in studies with adjudication based on AAOMS definition. Imaging exams are able to show real bone lesion involvement. The researchers reviewed CT scan of ONJ patients, classified at the exam time as "stage 0" according to AAOMS; we registered the main radiological aspects of ONJ sites and adjudicated the clinical radiological stage according to the Italian SIPMO-SICMF staging system. CT scans of maxilla and mandible were performed by spiral multidetector scanner. They examined 13 cases: M/F 6/7;breast cancer/prostate cancer/myeloma/renal cancer 5/4/3/1. CT pattern and stage by SIPMO-SICMF staging system: 4 sites showed focal sclerosis, for 1 stage Ia and 3 Ib; 10 sites showed diffuse sclerosis, with 4 stage IIa and 6 stage IIb; no alteration such as mandible fracture, skin fistula, sinus tract was detected.
