Multiple Allergen Simultaneous Test-Immunoblot Assay에서 Cross-reactive Carbohydrate Determinant 특이 항체에 의한 위양성 반응 방지: 증례 보고

교차 반응 탄수화물 결정 물질(cross-reactive carbohydrate determinant, CCD)은 아미노산 단백질 사슬에 부착된 단순 탄수화물이며, 식물, 곤충류, 벌목 독 등에서 발견된다. CCD 항원은 생체 내 생성된 항-CCD 항체와 광범위한 반응을 일으킬 수 있다. 50대 남성이 immunoblot 방법을 이용한 Multiple Allergen Simultaneous Test를 시행했다. PROTIA Allergy-Q 64 inhalant panel (ProteomeTech, Korea)에서 포유류와 관련된 알레르기항원을 제외한 59개 중 37개의 항원에서 양성 반응을 보여, 항-CCD 항체로 인한 교차 반응을 의심하였다. ProGlycAn CCD-blocker (ProGlycAn, Austria) 처리 후에는 CCD 항원과의 반응이 없었고 양성 반응을 보이는 알레르기항원의 수는 15개로 감소했다. 검체를 희석 처리만을 했을 때 교차 반응이 여전히 나타났다. 총 7명의 항-CCD 항체로 인한 교차 반응이 의심되는 환자를 추가로 CCD-blocker 처리를 시행했다. 이들 8명의 검체는 평균적으로 33개(범위 24-36개)의 알레르기항원이 양성으로 나타났으며, CCD-blocker 처리 후 양성 결과를 나타내는 알레르기항원 수는 평균 8개(범위 0-19개)로 감소했다. 항-CCD 항체로 인한 교차 반응이 의심되는 검체에서 CCD-blocker를 처리하는 것은 위양성 반응을 감소시켜 알레르기 원인 항원에 대한 특이적인 정보를 제공할 수 있을 것으로 기대한다. Cross-reactive carbohydrate determinants (CCDs) are simple carbohydrates linked to amino acid chains; they are found in pollens, vegetable foods, insect, and Hymenoptera venoms and are broadly cross-reactive with CCD-specific IgE antibodies. A man in his fifties was evaluated using a multiple allergen simultaneous test–immunoblot assay. On the PROTIA Allergy-Q 64 inhalant panel (ProteomeTech, Korea), reactions to 37 of 59 antigens were observed except mammalian antigens, and cross-reactivity owing to anti-CCD antibodies was suspected. After ProGlycAn CCD-blocker (ProGlycAn, Austria) treatment, the patient exhibited no response to CCD allergens, and the number of allergens showing positive reactions was reduced to 15. We further tested a total of 7 samples from patients who were suspected to have CCD-related cross-reactivity. For these 8 patients, the average number of positive reactions to allergens was reduced from 33 (range 24-36) to 8 (range 0-19) after CCD-blocker treatment. We concluded that CCD-blocker treatment in sample with anti-CCD antibodies can reduce the false positive reponse and provide more specific information about allergens. KCI Citation Count: 0
