Summary:Recently, the use of proton pump inhibitors in Korea has increased rapidly and the risk of pneumonia has beenraised in PPI users. We aimed to investigate the association between PPI and pneumonia and to identify the risk factors. In this case-control study, we used the National Health Insurance Sample Cohort Database (2002 to 2013). Cases weredefined as subjects hospitalized or visited emergency unit with pneumonia from January 2013 to December 2013, and upto four controls were matched to each case by age, gender among subjects without pneumonia. Conditional logisticregression was used to calculate the odds ratio (OR) and adjusted odds ratio (aOR) and 95% confidence intervals (CI). Atotal of 2,366 cases and 9,464 controls were included in this analysis. The crude odds ratio of PPI use and pneumoniawas 1.15 (95% CI, 1.01-1.30), and the association for most recent use of PPI within 30 days before hospitalization withpneumonia became stronger (OR, 1.33; 95% CI, 1.04-1.70). Comorbidities adjusted odds ratio was 1.15 (95% CI, 1.01-1.32), but aOR in recent PPI users was not statistically significant (aOR, 1.18; 95% CI, 0.90-1.54). This studydemonstrated that PPI use is associated with increased risk of pneumonia, but further studies reflecting patient's clinicalcondition and precious PPI use are necessary.
