Summary:목적: 본연구는일상급종합병원소아중환자실에서기존의 8시간교대근무와 2012년 3월부터새롭게도입된 12시간교대근무시행후의간호사들의직무만족도, 삶의질, 환자안전사건보고율및시간외근무시간을비교하기위함이다. 방법: A병원의소아중환자실간호사 36명을편의표집하여, 12시간교대근무전, 후직무만족도, 삶의질에대한자기기입식설문조사를시행하였으며환자안전사건보고율및시간외근무정도를분석및비교하였다. 결과: 환자중증도, 간호사와환자의비율, 급여및조직의제도적변화없이 12시간교대근무적용후직무만족도는적용전 3.49±0.30에서적용후 3.65±0.25로(t=3.93, p=<.001), 삶의질은적용전평균 2.83±0.34, 적용후 3.24±0.35로(t=7.83, p<.001) 통계적으로유의하게증가하였다. 6개월동안의1000중환자실재원일당환자안전사건보고율은근무형태변경전14.8건에서변경후13.9건으로감소하였지만통계적인유의성은없었고(χ2=0.15, p=.720), 시간외근무는변경전 36.3±34.7분, 변경후 17.3±34.9분으로감소하였다(Z=-8.91, p<.001). 결론: 12시간교대근무는환자안전사건보고율의증가없이간호사의시간외근무를줄이고,간호사의직무만족도와삶의질에긍정적인영향을미치며,유연근무제의한형태로의적용가능성을확인하였다. 12시간교대근무형태에대한간호의질, 간호사의업무중기민성및업무시간에따른신체적피로도에대한추후연구를제언한다. Purpose: The aim of this study was to compare job satisfaction, quality of life (QOL), incident report rate and overtimehours for 12-hour shifts and for 8-hour shifts in a pediatric intensive care unit (PICU). Methods: A descriptive surveywas conducted with a convenience sample of 36 staff nurses from a PICU in a regional hospital in Korea. Data werecollected using self-administrated questionnaires regarding job satisfaction and QOL at 6 months before and afterthe beginning of 12-hour shifts. Incident report rate and overtime hours for both 12-hour and 8-hour shifts werecompared. Comparisons were made using x2-test, paired t-test and Mann-Whitney U test. Results: After 12-hourshifts were initiated, job satisfaction significantly increased (t=3.93, p<.001) and QOL was higher for nurseson 12-hour shifts compared to 8-hour (t=7.83, p<.001). There was no statistically significant change in incidentreport rate (x2=0.15, p=.720). The overtimes decreased from 36.3±34.7 to 17.3±34.9 minutes (Z=-8.91, p<.001). Conclusion: These results provide evidence that 12-hour shifts can be an effective ways of scheduling for staff nursesto increase job satisfaction and quality of life without increasing patient safety incidents or prolonged overtime workhours.
