반월연골판 봉합술

반월연골판의 파열은 다양한 원인으로 발생하며 슬관절에 있어 수술적 치료를 요하는 가장 흔한 질환 중 하나이다. 반월연골판은슬관절에서 중요한 역할을 하며, 그 기능은 하중 전달, 충격 흡수, 슬관절 안정화, 고유감각 제공, 윤활 및 영양 공급에 이르기까지다양하다. 오래전에는 반월연골판 파열에 대해 반월연골판 절제술이 주로 사용되었으나, 장기적으로 퇴행성 변화의 증가가 보고되어 반월연골판의 보존에 초점을 맞춘 반월연골판 봉합술이 치료의 우선 순위로 자리잡게 되었다. 반월연골판 파열에 있어 파열의 위치, 유형 등에 따라 다양한 수술 기법들이 시행되고 있으며, 반월연골판 봉합을 통한 치료의 결과에는 다양한 요인들이 영향을 미친다. 본 종설은 반월연골판 봉합술의 적응증과 수술 기법, 발생할 수 있는 합병증 및 수술 후 재활 방법에 대해 정리하여 제시하고자 한다. 또한 최근 시도되고 있는 반월연골판 보강술 및 치료의 발달로 인해 확장된 적응증에 대한 내용을 통해 반월연골판 봉합술에 대한 최신 지견을 전달하고자 하였다. Meniscal tears are one of the most common injuries of the knee joint from various causes requiring surgical treatment. The meniscus plays a crucial role in the knee joint, with functions ranging from load transmission and shock absorption to stabilizing the knee and providing proprioception, lubrication, and nutrient supply. While a meniscectomy was commonly performed in the past for meniscus tears, long-term studies have shown an increase in degenerative changes, leading to a priority shift towards meniscal preservation through meniscal repair surgeries. The surgical techniques vary according to the type and location of the tear, and the outcomes of meniscal repair are influenced by various factors. This review article discusses the indications for meniscal repair, surgical techniques, potential complications, and postoperative rehabilitation. In addition, it seeks to provide the latest insights on meniscal repair by discussing the expanded indications resulting from recent advances in meniscal augmentation and treatment development.
