Summary:The Thromboangiitis obliterans is an obstructive arterial disease that is characterized by proliferatives inflammatory injuries you would segment of the medium and small arteries and of the superficial veins of the extremities. The presentation of a patient of 43 years of carrying age for 15 years of this disease has been made, ex-smoking, to which of it made bilateral lumbar sympathectomy to him and in spite of it it evolved towards the necrosis and ulceration of the dístals portions of the first right toe. Transphalangical amputation of he himself was made to him subsequent to and complication it with venous Thrombosis iliofemoral right and pulmonary thromboembolism of fine branch with favorable final evolution. The atypical of this case is expressed by: The little frequency of this disease in feminine sex, its unfavorable evolution towards the occlusion and ischaemia in spite of the abandonment of the habit to smoke on the part of the patient and of the made bilateral sympathectomy, its complication with deep a venous thrombosis, which is little frequent and with a pulmonary thromboembolism of fine branch with the satisfactory evolution of this last process in spite of the high well-known morbi-mortality of he himself. Introducción. La Tromboangitis Obliterante es una enfermedad arterial obstructiva que se caracteriza por lesiones inflamatorias proliferativas segmentarías de las arterias medianas y pequeñas y de las venas superficiales de las extremidades. Se realiza la presentación de una paciente de 43 años de edad portadora desde hace 15 años de esta enfermedad, exfumadora, a la cual de le realizó Simpatectomía lumbar bilateral y a pesar de ello evolucionó hacia la necrosis y ulceración de las porciones dístales del primer dedo del pie derecho. Se le realizó amputación transfalángica del mismo y posterior a ello se complica con Trombosis venosa ileofemoral derecha y Tromboembolismo pulmonar de rama fina con evolución final favorable. La atipicidad de este caso se expresa por: la poca frecuencia de esta enfermedad en el sexo femenino, su evolución desfavorable hacia la oclusión e isquemia a pesar del abandono del hábito de fumar por parte de la paciente y de la Simpatectomía bilateral realizada, su complicación con una trombosis venosa profunda, lo cual es poco frecuente y con un tromboembolismo pulmonar de rama fina con la evolución satisfactoria de este último proceso a pesar de la elevada morbimortalidad conocida del mismo.
