증 례 : 자가면역성 췌장염과 감별이 어려웠던 췌장 림프종 1예

최근 우리나라에서는 자가면역췌장염의 증례보고가 늘고 있다. 자가면역췌장염의 정확한 진단은 임상적으로 매우 중요한데, 이는 임상적으로 그리고 영상학적으로 췌장 악성종양과의 감별이 어렵기 때문이다. 자가면역췌장염의 경우, 스테로이드에 대하여 반응이 좋고, 수술적 치료를 피할 수 있다는 점에서 그 의의가 크다. 이번 증례보고에서 저자들은 영상검사에서 자가면역췌장염을 충분히 의심할 만한 환자에서 생검상 림프종으로 확진된 증례를 경험하였기에, 문헌 고찰과 함께 보고하는 바이다. Accurate diagnosis of autoimmune pancreatitis (AIP) is important to clinicians since it is difficult to differentiate AIP from pancreatic malignancies. Furthermore, unlike pancreatic malignancies, AIP has dramatic response to steroids. A 61-years-old man presented with acute pancreatitis. Imaging studies showed two separate pancreatic masses, irregular narrowing of main pancreatic duct, and a renal mass that highly suggested AIP. Endoscopic ultrasound-guided core needle biopsy of the pancreatic masses and ultrasound-guided biopsy of the renal mass revealed peripheral T-cell lymphoma. The patient is currently undergoing chemotherapy. We present a case of pancreatic lymphoma masquerading as AIP with literature review.
