대롱유두샘종과 병발한 바터팽대부 림프용종

림프용종은 림프 조직이 국소적 또는 미만성으로 증식하여 발생하는 양성질환으로, 림프구 증식(lymphoid hyperplasia)혹은 양성 림프종이라고 불리기도 한다. 잘 분화된 림프구로 구성되어 있으며, 선명한 배중심과 난포 패턴을 보이는 정상적인 림프 조직의 증식에 의해서 악성 림프종과 구분할 수 있다. 소아에서 상대적으로 흔하게 발생하고 성인에서는 직장에 드물게 발생한다. 저자들은 황달, 체중감소, 전신 쇠약감 등의 증상으로 발현된 바터팽대부의 종양을 내시경 유두 절제술을 시행하여 대롱 유두샘종과 병발한 림프용종으로 진단한 1예를 경험하여 문헌고찰과 함께 보고한다. Lymphoid polyp is a very rare disease that commonly occurs in the rectum. It is a benign, focal or diffuse lesion that typically occurs where clusters of lymphoid follicles are present. The polyp is composed of well differentiated lymphoid tissue. It can generally be differentiated from malignant lymphoma by the proliferation of normal lymphoid tissue, which has a prominent follicular pattern and a clearly defined germinal center. There have been only a few reports of lymphoid polyps of the rectum, and there have been no reports of lymphoid polyp in the ampulla of Vater. We experienced a case of lymphoid polyp in the ampulla of Vater associated with tubulopapillary adenoma, and the patient first presented with jaundice, weight loss and general weakness.
