내시경적 용종절제술 후 발열의 소실을 보인 대장결핵 1예

대장결핵은 임상적, 방사선학적, 대장내시경적 소견이 비특이적이고 악성 종양과 유사하여 진단이 어렵다. 결핵 환자의 약 60∼85%에서 발열이 있고, 이것은 결핵의 활동성을 나타내는 중요한 징후 중의 하나이다. 항결핵제를 투여한 후 2주 이내에 발열은 대부분 소실되나, 용종양 병변을 가진 대장결핵 환자에서 항결핵제 투여 후에도 상당한 기간이상 발열이 지속되거나 치료 중에 발열이 생기는 경우에는 육아종으로부터 지속적인 사이토카인의 유리에 의한 발열, 약물 유발성 발열, 약물내성, 약물 흡수장애 등을 의심해야한다. 저자들은 장기간의 발열이 있었던 용종양 병변이 동반된 대장결핵 환자에서 내시경적 용종절제술후 신속하게 전신증상이 호전되고 발열이 소실된 대장 결핵 1예를 경험하여 문헌고찰과 함께 보고하는 바이다. The clinical manifestations, radiological and endoscopic findings of colonic tuberculosis are non-specific. A diagnosis of colonic tuberculosis is usually difficult because the condition can mimic tumors. Fever occurs in 60∼85% of patients with tuberculosis, which is one of the important signs of disease activity, and usually resolves by the second week after beginning treatment. However, there are some patients who remain febrile beyond a reasonable treatment period or develop fever during treatment. Such cases raise issues, such as cytokine release from tuberculous granuloma, drug induced fever, drug resistance, and drug malabsorption. We encountered a patient with polypoid colonic tuberculosis who presented with prolonged fever after commencing treatment and became defervescence after a colonoscopic polypectomy. We report this case with a review of the relevant literature. (Korean J Gastrointest Endosc 2008;36:31-35)
