Jakob 2형의 소아 상완골 외과 골절에서 관혈적 고정술과 폐쇄적 경피 핀 고정술의 비교

목적: Jakob stage II의 소아 상완골 외과 골절에서 관혈적 정복 고정술과 폐쇄적 경피 핀 고정술의 치료 결과를 비교하고자 하였다. 대상 및 방법: 2000년 2월부터 2004년 12월까지 Jakob stage II의 소아 상완골 외과 골절로 수술치료를 받은 환자 21명을 대상으로 하였다. 이 21명 중 11명은 폐쇄적 경피 핀 고정술을 시행하였으며 10명은 관혈적 정복을 시행하였다. 이 두 군을 대상으로 최종 추시 상의 관절 운동범위, 운반각, 반흔 등 외형상의 변화 및 방사선 검사 결과를 비교하였다. 결과: 주관절운동 범위는 관혈적 정복군의 경우는 환측이 건측에 비해 평균 3.8도 감소한 반면 폐쇄적 핀 고정군에서는 양측간 차이를 보이지 않았다. 운반각은 두 군 모두에서 양측간 1도 이내의 차이를 보였다. 관혈적 정복군의 경우 평균 5.2 cm의 창상반흔이 남아 반흔에 대한 불만이 높았던 반면 경피적 핀 고정군에서는 모두 만족스러운 결과를 얻었다. 방사선 검사상 두 군 간에 현저한 차이는 볼 수 없었다. 결론: Jakob stage II의 소아 상완골 외과 굴절에서 폐쇄적 경피 핀 고정술은 관혈적 정복에 비해 기능적으로나 방사선적으로 차이가 없는 양호한 결과를 얻었으며 반흔에 대해서는 훨씬 만족스러웠으므로 보다 바람직한 치료로 생각되었다. Purpose: To compare the results of open fixation and closed percutaneous pinning in managing Jakob stage II lateral condylar fractures of children`s elbow. Materials and Methods: Since Febuary 2000, We operated 21 children with Jakob stage II lateral condylar fractures of elbow. Eleven of the 21 were treated with closed percutaneous pinning, open fixation was done to the other 10 children. Each patient was evaluated about range of motion, carrying angle, scar satisfaction and radiologic findings for comparison between closed pinning and open fixation groups. Results: Open fixation group showed 3.8° decrease of elbow motion while closed pinning group showed no significant decrease. Carrying angle and radiologic findings were not different between the two groups. Open fixation group expressed dissatisfaction to their scars (average 5.2 cm) whereas all the patients of closed pinning group were satisfied with their functional and cosmetic outcomes. Conclusion: In managing Jakob stage II lateral condyle fractures of children`s elbow, closed percutaneous pinning was thought to be superior to open fixation because of the same functional outcome and much better cosmetic results.
