Summary:Eight cases of hyperosmolar hyperglycemic non- ketotic coma (HHNC) were studied clinically at Chon- buk National University Hospital during the period of 10 years from 1981 to 1991. The incidence of the HHNC among the diabetes associated with acute metabolic complicatin (51 cases) were 15.7%. The patient's age were over fifty year-old (75%) and the sex were female predominant (75%). The chief complaints of patients were disturbance of con- sciousness (75%) and all patients had signs of dehydra- tion (100%). The major precipitation factors was infection (50%) and the others were inappropriate treatment (37.5%), nonspecific GI disturbances (25%). Initially diagnosed diabetes mellitus by episode of HHNC were 62.5%. Iaboratory findings were leukocyte average 15,600/ul, PMNL 80%, Ph 7. 39± 2. 7, HCO, 21. 8 ± l. 7 mEq/l and anion gap 17.1±2.5 mEq/1. The significantly high levels of serum osmolarity and Na' were observed in patient with disturbance of consciousness. On admission hypog- lycemia and hyponatremia was observed 50% each, and motality rate was 12.5%.
