Summary:Virilization may occur during pregnancy as the result of an ovarian Krukenberg tumor. Mechanism of the androgen over-production in this exceptional condition is still poorly understood. A new case is reported in which clinical, endocrine and endoscopic studies led to the diagnosis of an ovarian Krukenberg tumor secondary to a gastric carcinoma. The mother and female infant appeared virilization. In the hormonal studies, testosterone and DHEA-S were increased markedly before orphorectomy and returned to almost normal 7 days after surgical removal. The testosterone level was also elevated right after birth, but returned to normal 7 days later in the infant. The upper G-I series and endoscopic studies showed advancd cancer in the mid-body of the stomach. At microscopic examination, numerous typical signet rigng cell were observed both in the endoscopic biopsy of the gastric tumor and the ovaries. In the ovaries there were also profound stromal cell hyperplasia. The role of metastatic ovarian cancer to develop virilization in pregnent woman was discussed.
