Backgrouad: Korea is known to have the lowest incidence rate of insulin dependent diabetes mellitus(IDDM) in the world and many of the adult IDDM patients show atypical clinical characteristics, namely, slowly progressive IDDM. However, clinical and epidemiological features of childhood diabetes have not yet been precisely evaluated. The aim of the present study is to elucidate the clinical characteristics of childhood IDDM in Korea. Method: We analyzed the clinical data of 92 (41 males, 0.25-16 years) diabetic children referred to Seoul National University Children's Hospital retrospectively. Results : All patients were diagnosed before the age of 16 and the duration of disease at the time of visit were 0.93+1.57 years. The mean age of onset was 7.5 years(45days-16 years) and we found 3 peaks, at the age of 7, 10 and 13 years. Body mass indexes at the time of diagnosis were 15.5+1.9 kg/m and mean linear growth of these patients follows the 50th-60th percentile. The monthly incidence pattern shows peak incidence in winter and early spring and lowest rates occurring in summer. Diabetic ketoacidosis was noted in 30.4% of the patients, Fasting plasma C-peptide levels were 0.09+0.08 nmol/1,24hr urinary excretion of C-peptide was 5.92+9.21 nmol and mean daily insulin requirements were 0.79+0.27 units/kg. HLA analysis showed a positive association with DR 3 and 4 antigens and a negative associaion with DR 2 antigens. Conclusion : Despite extremely low incidence and atypical characteristics in adult IDDM, the clinical characteristics of childhood IDDM in Korea are quite similar to those of Caucasians. |
