Summary:Posterior interosseöz sinir (PİS) radial sinirin saf motor dalıdır. Posterior interosseous sinir kompresyon sendromu ise radial sinirin derin dalının dirsek ekleminin hemen distalinde gelişen tuzak nöropatisidir. Bu sendrom klinik olarak düşük bilek olarak adlandırılan, ön kolda saf ekstansör kas güçsüzlüğü ile karakterizedir. Hastaların şikâyeti başparmak güç- süzlüğü ve eli kullanmada güçlüktür. Genellikle radius kemiğinin kırık ve çıkıkları, önkola sık enjeksiyon yapılması, bazı eksternal travmalar, tümöral oluşumlar, dirsek eklemi de- formiteleri, fibröz bantların, intermusküler septaların, skar veya adezyonlarının yaptığı bası ve tekrarlayıcı önkol supinasyon ve pronasyon hareketleri nedeniyle ortaya çıkabilir. Bazen herhangi bir neden ortaya konamayabilir. Bu yazıda 46 yaşındaki bir ebede ani ge- lişen ve nedeni ortaya konamayan bir posterior interosseöz sinir kompresyon sendromu olgusu sunulmakta ve tanıda elektronöromyografi (ENMG)’nin önemi belirtilmektedir.Anahtar kelimeler: Sinir kompresyon sendromu, Radial sinir, ElektromyografiABSTRACTPosterior interosseous nerve (PIN) is the pure motor branch of the radial nerve. Posterior interosseous nerve compression syndrome (PINCS) is an entrapment neuropathy resulting from the compression of the deep branch of the radial nerve just distal to the elbow joint. This syndrome clinically characterized by the pure extensor muscle weakness of the forearm, called as wrist drop. The major compliant of the patients is the difficulty in the use of the thumb and hand. This syndrome usually results from the fracture or dislocations of radius, frequent injections to the forearm, infections, some external traumas, tumors, elbow joint deformities, fibrous bands, pressure of intermuscular septa, scar or adhesions, and repetitive forearm supination and pronation movements. Sometimes underlying factor may not be found. In this report, we are presenting a new case of PINCS in a 46 years old midwife and discussing the importance of electromyography in the diagnosis of this syndrome.Key words: Nerve compression syndrome, Radial nerve, Electromyography Posterior interosseous nerve (PIN) is the pure motor branch of the radial nerve.
