Summary: | The present disclosure relates to the patient monitoring arts and provides systems and methods of monitoring one or more patients that are transitioning from one physical environment to another physical environment. The patient monitoring systems of the present disclosure include an interface device comprising one or more universal ports and a networking component, wherein the interface device is configured to receive sensor data from one or more physiological sensors attached to the patient via one or more smart cables, and wherein the networking component is configured to transmit the sensor data to a remote data storage unit. In accordance with the systems and methods of the present disclosure, a patient may be seamlessly transitioned from a typically high-acuity environment to a typically low-acuity environment.
