Disclosed is a form processing device and recording medium that can specify the position where each item is written within an input image and process each item precisely and at high speed, even when a form image has been input using an image input device such as a camera whereby the form image capture conditions are unstable. For forms in which the target processing region is surrounded by a uniform blank area and is sandwiched between a start code and an end code, which express the direction and height of the target processing region, a region code detection unit detects the start code and the end code from the input image; a region extraction unit extracts a target processing region by using the positions, heights and directions of the start code and the end code; and a processing unit processing the contents written in the target processing region in accordance with the start code and end code types; thereby extracting the start and end codes for items in the input image of a form to identify the position within the input image where each item is written, and processing each item.
