The ensemble prediction model accounts for effects of feature combinations on outcomes for the users. The present system is configured such that output from the ensemble prediction model is used during a single agent search to determine optimal combinations of services that minimize the risk of emergency re-hospitalization and/or other negative patient outcomes.
