The present disclosure provides a wound care assessment system includes an image capturing device and a processor. The image capturing device is for capturing a target wound image data of a subject. The processor is connected to the image capturing device electronically and signally, and the processor includes an image labeling module, a data preprocessing module and a wound grade assessment model. The image annotation module is used for receiving and analyzing the target wound image data. The data preprocessing module is connected to the image labeling module signally. The wound grade assessment model is connected to the data preprocessing module. The wound care assessment system is to assess a grade of pressure injury and a wound bed care index of a wound based on the target wound image data. Therefore, an accurate and real-time information on wound grading can be provided to front-line medical staffs.
