Summary:The present invention discloses a built-in speed-shifting mechanism of medical care assistive device, which includes a main shaft body, a speed-adjusting shaft, a driving shaft and a ring gear. By controlling the rotation of the speed-adjusting shaft, a plurality of driving/fixed clutches correspondingly installed are configured in an engaged or a disengaged state, so as to determine whether the action of the driving/fixed clutches achieves a plurality of gear shifts. The speed-shifting mechanism of the present invention can be coupled to vehicle wheels to generate different power transmission paths (such as high gear, direct drive gear, neutral gear, reverse gear, etc.) according to the requirement of an operator, so as to drive the vehicle wheels to move in various terrains. In addition, all of said components are designed inside a casing (hub), and thus they are not easily affected by external environmental factors (such as external force collision, dust contamination, environmental moisture, etc.), thereb
