SUBSTANCE: given laser perforator has laser radiator, optical forming system incorporating cylindrical and spherical lenses, optical system for illumination and positioning coming in the form of mutually complementary mechanical and optical parts. Optical system for illumination and positioning includes condenser, source of visible radiation, lens, deviating element and deviating beam splitter mounted in sequence on optical axis, it is supplemented with combined light filter and diaphragm placed between source of visible radiation and lens. Light filter has color differing from color of patient's skin. Support with blind hole whose bottom is located outside of plane of focusing of laser radiation is added in point of focusing of laser radiation. Axis of blind hole is uniaxial with axis of forming optical system. Support is made of material absorbing defocused laser radiation. Technical result of invention lies in precise positioning of focusing plane on skin surface, in obtainment of perforation wound in the form of cut having width not more than 0.3 mm and length of 1.0-3.0 mm, in prevention of laser radiation from hitting eyes of operator or patient if patient's finger is not present in point of focusing of laser radiation.
