Disclosed are an apparatus for forming a Bragg grating on an optical fiber having a large diameter and a method thereof. According to one embodiment of the present invention, the apparatus comprises: an optical fiber moving module configured to move an optical fiber to a jig and fix the optical fiber to a fixing/moving jig; a phase mask moving module configured to move a phase mask having a predetermined pattern to the fixing/moving jig and fix the phase mask to a mounting groove of the fixing/moving jig; a jig inclination angle adjusting module configured to adjust an inclination angle of the fixing/moving jig to move the optical fiber in a + z axis or -z axis direction; a jig moving module configured to move the fixing/moving jig in a ± x axis or ± y axis direction; an optical module configured to irradiate a laser beam to the phase mask as the jig inclination angle adjusting module and the jig moving module adjust a moving direction of the optical fiber; and a focusing module configured to collect the laser beam irradiated from the optical module.
