To provide a heat exchanger exerting high heat exchange performance, and a medium system.SOLUTION: A heat exchanger 30 is equipped with a flow control element in a parallel passage 50. The flow control element is disposed at a terminal end portion of the parallel passage 50. The flow control element includes any one of a dome surface 54, an inclined surface 56, a projecting surface 64, or a blade 69. The flow control element may include two or more of the dome surface 54, the inclined surface 56, the projecting surface 64, or the blade 69. The flow control element adds vertical flowing components to a flow of a first medium in the parallel passage 50. As a result of that, immediately after flowing into a first distribution passage 31b, in a wide range extending to a terminal end of the first distribution passage 31b, a first medium is distributed to a plurality of first heat exchange passages 31a without significant unevenness.
