To provide an information recording laminate with a seal for normally hiding a point written with a piece of information and for allowing a person to peel off the seal and read the information at an emergency case for enabling a proper treatment based on the information, and information presentation device using the information recording laminate.SOLUTION: The information recording laminate comprises a base material sheet, an adhesive layer, and a release paper layer. The base material is configured so as to, when being folded into two, one end or both ends are exposed without being overlapped with each other. On the surface of the release paper layer, information recording area is formed for recording a piece of information. The release paper layer is formed with an incision along a line enclosing the information recording area. The base material is formed with an opening area. On the surface of the base material, cover area is detachably provided for covering the opening area. On the surface of the base material, the information recording area is formed so as to be positioned in the opening area when the release paper layer is folded into two.SELECTED DRAWING: Figure 1 【課題】本発明は、通常時は情報を書き込んだ箇所をシールで隠しておき、緊急時に周囲の人間がそのシールを剥離し情報を読むことで、情報に基づいて適正な処置を行うことを可能とする情報記載用積層体及び情報記載用積層体を用いた情報表示具を提供する。【解決手段】シート状の基材、粘着剤層、及び剥離紙層とからなり、基材は真二つに折返したときに、一端または両端が重ならず露出し、前記剥離紙層の表面には情報を記載するための情報記載領域部が設けられ、前記剥離紙層には、前記情報記載領域部を取り囲む線に沿うように切込み部が設けられ、前記基材には、開口部領域が設けられ、前記基材の表面には、開口部領域を被覆するカバー領域部が着脱可能に設けられ、前記基材の表面には、前記情報記載領域部は、剥離紙層を真二つに折り返したときに前記開口領域部内に位置するように設けられることを特徴とする情報記載用積層体とする。【選択図】 図１
