PROBLEM TO BE SOLVED: To provide a recording form for a concrete test piece that has high workability and can be easily pasted at a determined position.SOLUTION: A recording form 1 for a concrete test piece on which information about the concrete test piece is described is provided. Then, the recording form 1 includes: a description layer including a description part on which the information is described on the front side facing an inner peripheral surface 9b of a frame 9 used for molding the test piece; arm units 5 and 5 extended upward from the upper edge of the description layer; and mounting units 51 and 51 that are formed in the direction substantially orthogonal to the tips of the arm units and are installed in an upper edge 9a of the frame. 【課題】作業性に優れるうえに、決められた位置に容易に貼り付けることが可能なコンクリート供試体用の記録票を提供する。【解決手段】コンクリート供試体に関する情報が記載されるコンクリート供試体用の記録票１である。そして、供試体を成形する際に使用される型枠９の内周面９ｂに対峙させる表面側に情報が記載される記載部が設けられる記載層と、記載層の上縁から上方に延伸されるアーム部５，５と、アーム部の先端に略直交する方向に向けて形成されて型枠の上縁部９ａに設置される載置部５１，５１とを備えている。【選択図】図１
