PROBLEM TO BE SOLVED: To provide a form retrieval device requiring less labor in preliminary works, when retrieving forms from among a plurality of forms by limiting retrieval objects to headline items. SOLUTION: A form retrieval device 1 includes a form data information storing unit 31 for storing a plurality of character strings in a form and drawing positions of the character strings for each form; a keyword extracting unit 21 for extracting character strings and their drawing positions including retrieval keywords, constituting a part of conditions for retrieving the form from among character strings in each stored form; a headline candidate retrieval unit 22 for retrieving headline candidates, corresponding to the character strings from among the forms from which the character strings are extracted, on the basis of the drawing positions of the extracted character strings; an output unit 40 generating headline candidate information which associates, for each form, the retrieved headline candidates to the character strings corresponding to the headline candidates; and a form retrieval unit 24 retrieving the character strings related to the headline candidates selected by a predetermined operation from the form data information storage unit 31 from among the headline candidate information output.
