PROBLEM TO BE SOLVED: To solve the following problem: since an opening history showing opening of a cover of a taking-out port for a recording medium can be easily erased in the recording medium housed in a cartridge, recording is performed to the recording medium which is once taken out and has dirt and thereby highly accurate recording operation sometimes is not performed. SOLUTION: The recording medium includes a cartridge case and a disk provided in the cartridge case. In the recording medium, the cartridge case has an information writing port and a disk taking-out port, the disk taking-out port is freely openably/closably covered by a cover and a displacement body leaving history of opening and closing operations of the cover is provided in the vicinity of the inner part of the disk taking-out port.
