PROBLEM TO BE SOLVED: To provide a folding form which has a great appeal effect and is easily adapted to a change in advertising claim and manufacturing situation. SOLUTION: This folding form 10 is equipped with: a form base material 11 which has first to eighth information areas 11-1 to 11-8 partitioned by first to seventh folded parts 12-1 to 12-7, and wherein the areas from the area 11-8 on a leading end side, folded and arranged near the center, to the area 11-8 on a terminal end side are sequentially wound and folded; a first sealing adhesion layer 13 for sealing a part near the terminal end of the wound and folded base material 11; an unsealing perforation 15 for unsealing the folding form 10 by cutting six places from the folded parts 12-2 to the terminal end of the base material 11; a second sealing adhesion layer 14 which is provided on a part or the whole of the vicinity of the top and bottom of the base material 11 so as to seal the wound and folded base material 11; and the like.
