Summary:PROBLEM TO BE SOLVED: To provide a bag-making form which enables issuance of a leaflet or the like without necessitating enclosing and sealing operations and an operation of sticking an address label and which is easy to store and easy to see. SOLUTION: The bag-making form 10 of an integrated type with a book is used as a user's manual and certificate, and it has one or more booklet slips 11-1 to 11-n constituting a booklet 11, a front cover slip 12 and a back cover slip 13 holding the booklet slips 11-1 to 11-n from the front and the back, a part 14 binding spines of the cover slips 12 and 13 and spines of the booklet slips 11-1 to 11-n, a part 15 sticking together the peripheral edge parts other than the spines of the cover slips 12 and 13, perforations (prescribed cutoff lines) 16 for cutting off the sticking part 15 for unsealing, etc. COPYRIGHT: (C)2004,JPO
