Summary:PROBLEM TO BE SOLVED: To provide a delivery slip being easy to manufacture and excellent in handling and economical properties. SOLUTION: The delivery slip is composed of a base 1 constituted of a paper or resin film which is formed in an appropriate shape and makes it possible to see through and of one slip sheet 4 formed of a plurality of slips 4a, 4b and 4c so joined continuously as to be capable of being cut off from each other. An adhesive layer 2 is provided in the periphery of one surface of the base 1 and further an opening 3 for throwing-in of the slip sheet 4 is formed in the portion of the base 1 inside the adhesive layer 2 where the adhesive layer 2 is not provided. In the rear of the slip sheet 4, a release agent layer 6 is so provided as to correspond at least to the adhesive layer 2 provided on the base 1, and the base 1 and the slip sheet 4 are stuck to be integrated so that the adhesive layer 2 and the release agent layer 6 are laid on each other.
