The invention discloses a test device and a test method for simulating underground hot water calcification formation. The test device comprises a pressurization carbon dioxide and water introduction module, a water bath heating module, a model cylinder module, a TDS, an ion monitoring module and a calcification formation observation module. The pressurizing carbon dioxide and water introducing module comprises a CO2 gas cylinder, a high-pressure pump, a mixing box body and a water conveying hose; the water bath heating module comprises a water tank module, an electric hot plate heating module, a temperature control module and a temperature sensor. The model cylinder module comprises a hard water conveying pipe, a cylindrical barrel, a sealing cover, a base, a modified test material filled in the barrel, a drainage pipe and a drainage valve. And the TDS and ion monitoring module comprises a TDS electrode and a corresponding circuit control part. The calcification formation observation module comprises a coolin
