The invention discloses a surgical microscope and an ophthalmic system. A surgical microscope includes an objective lens, a first illumination optical system, a deflection unit, and an observation optical system. The first illumination optical system is configured so as to be substantially coaxial with the optical axis of the objective lens, and is capable of irradiating the first illumination light to the eye to be operated via the objective lens. The deflection unit deflects return light of the first illumination light incident from the eye to be operated via the objective lens in a direction intersecting the optical axis.
