The invention provides an Internet protocol television (IPTV) quality of service guarantee method and a media server. The method comprises the following steps of: judging the level of a corresponding user after the media server authenticates a media play request from a set top box; when determining that the user is the common user, transmitting a corresponding media stream to a set top box of thecommon user in a standard media stream transmission mode; when determining that the user is an advanced user, transmitting a corresponding media stream to a set top box of the advanced user in the standard media stream transmission mode; monitoring a packet loss rate of a media stream which is played by the set top box of the advanced user currently; and when the packet loss rate is higher than apreset value, transmitting a corresponding media stream to the set top box of the advanced user in a multiple media stream transmission mode.
