Summary: | A 67-year-old Caucasian female patient visited the Oral Diagnosis Center with heterogeneous white patches present on the palate, buccal mucosa, and dorsal tongue. An incisional biopsy of the palatal lesion was conducted, revealing hyperkeratinized stratified squamous epithelium with broad projections and basal layer polarization, indicative of moderate epithelial dysplasia. Based on these findings and the presence of 3 major and 3 minor criteria recommended for diagnosis, the patient received a diagnosis of proliferative verrucous leukoplakia (PVL). Throughout the previous 11 years of follow-up, the patient had displayed a reticular white lesion on bilateral buccal mucosa and desquamative gingivitis, which was histologically characterized by an interface mucositis pattern. Both the clinical and histopathological aspects led to the diagnosis of oral lichen planus (OLP). The progression of OLP lesions to PVL remains a subject of debate in the literature. However, these clinical characteristics suggest a heightened risk of malignant transformation, emphasizing the necessity of regular clinical follow-up and biopsies. |
