Summary:The central odontogenic fibroma is a rare benign neoplasm. An 18-year-old female patient was referred to the Oral and Maxillofacial Surgery service due to a radiographic finding. During the intraoral examination, the patient exhibited swelling in the posterior region of the left maxilla and tooth displacement. Computed tomography revealed a hypodense, expansive image with bone cortex destruction and displacement of adjacent teeth. An incisional biopsy was performed, and the histological report and immunohistochemical profile were consistent with odontogenic fibroma. The proposed treatment included enucleation/excision of the lesion and extraction of the affected teeth. After 8 months of follow-up, bone reconstruction was recommended using a mandibular oblique line graft. The patient is currently under observation with no signs of recurrence. Although odontogenic fibroma lacks a capsule, it can be easily separated, and conservative surgical treatment with thorough curettage has demonstrated favorable outcomes, with low recurrence rates.
