Summary:Abstract Oral sub mucous fibrosis is a commonly encountered clinical problem of the Indian sub-continent. Treating it with surgery and reconstruction with appropriate flap followed by vigorous physiotherapy results in the most ideal management. We report three advanced cases which were successfully managed and rehabilitated with bi-paddled radial forearm free flap. Pre-operative mouth opening of mean 3mm was surgically released and post-operative mouth opening of mean 27mm was achieved after 18months follow-up. In conclusion, we recommend the use of a single donor site, bipaddled radial forearm free flap, for reconstruction of bilateral buccal defect after oral submucous fibrosis release instead of two flaps.
