Summary:We are reporting a case of a pancreaticopleural fistula causing pain of the epigastrium. Chest radiograph revealed liquid in the pleural cavity that was reaching VII rib. Puncture of the pleural cavity has provided 1600 ml of brown liquid with an elevated level of diastase(s?). Ultimately, a definitive diagnosis was confirmed by computed tomography. Due to the failure of conservative and endoscopic methods, the decision to perform surgery was made. It resulted in total pancreatectomy.
