A case of eccrine porocarcinoma in an elderly woman is reported. A 78-year-old female presented with a reddish-brown plaque localized on her left lower leg for 8 years. Histopathological examination showed eosinophilic lobular masses that composited by poroma cell. The cells were presented with condensed cytoplasm and atypical hyperchromatic nuclei. Immunohistochemistry showed epithelial membrane antigen positive. The diagnoses eccrine porocarcinoma was established according to its clinical manifestations and histopathological changes. The patient received a surgery and was still in follow-up.
