Summary:The trend to preserve the spleen whenever possible after its blunt trauma forced the necessity of the workout of the differentiative approach to the problem. The most disputable situation is the formation of the laminar subcapsular hematoma of the spleen. The conservative treatment of such lesions can lead to the two-stage rupture with severe intraabdominal bleeding. Nevertheless, the results of the morphological analysis demonstrate the early beginning of reparative process around the hematoma, securing the possibility of the septic inflammation.
