Summary:Isolated fractures of the medial malleolus have been classified recently by Herscovici into four types. Most of these injuries can be managed conservatively. Surgical management depends upon the fracture anatomy. We describe the surgical technique and postoperative management of a patient with isolated fracture of the medial malleolus with an intra-articular step. Management requires careful evaluation of ankle stability and soft tissues around the ankle before making a decision.
