장기간 스테로이드를 사용한 청소년기 중증 근무력증 환자에서 발생한 골다공증성 다분절 흉요추부 압박골절에 대한 테리파라타이드(Teriparatide) 치료

중증 근무력증은 신경근 접합부의 자가항체에 의해 근육의 감소 및 파괴를 일으키는 자가면역 질환으로, 스테로이드가 일차 면역억제 치료제이다. 스테로이드는 면역체계에 영향을 미쳐 대부분의 중증 근무력증의 아형에서 효과를 보이므로 유지 치료를 위해장기간 사용하는 경우가 많고, 그로 인한 골밀도 감소 및 골절 위험성 증가 등 부작용이 발생할 수 있다. 저자들은 중증 근무력증을 진단받은 후 장기간 스테로이드를 사용 중인 18세 여환에서 외상없이 발생한 골다공증성 다분절 흉요추부 압박골절에 대하여테리파라타이드(teriparatide)를 이용하여 만족할 만한 치료 결과를 경험하였고, 청소년기 중증 근무력증 환자에서 장기간 스테로이드 사용으로 인해 발생한 골다공증성 다분절 흉요추부 압박골절과 치료는 문헌상 보고된 바가 없기에 이를 문헌 고찰과 함께 보고하고자 한다. Myasthenia gravis is an autoimmune disease in which autoantibodies affect the neuromuscular junction, leading to muscle loss. Steroids are the primary immunosuppressive treatment for this condition. Since steroids affect the immune system and are effective in most subtypes of myasthenia gravis, they are often used for long-term maintenance treatment. This increases the probability of steroidassociated side effects such as decreases in bone mineral density and increased risk of fractures. The authors successfully treated an 18-year-old female who had been using steroids for a long time after being diagnosed with myasthenia gravis using teriparatide for osteoporotic multi-segment thoracolumbar compression fractures that occurred without trauma with satisfactory outcomes. Since there have been no reports of osteoporotic multi-segmental thoracolumbar compression fractures and treatment in juvenile myasthenia gravis patients caused by long-term steroid use, we report this case along with a review of the literature.
