갈고리 금속판을 이용한 제5형 원위 쇄골 골절의 치료 이후 발생한 쇄골 간부의 금속판 주위 골절: 증례보고 2예

원위 쇄골 골절 혹은 견봉-쇄골관절 탈구에 많이 사용되는 갈고리 금속판은 좋은 임상적인 결과를 보임에도 불구하고, 금속판의구조적 특성 또는 술기와 관련한 몇몇 합병증들이 발생할 수 있으며, 견봉골절 혹은 회전근개 파열과 같은 금속판 원위부의 문제가 많이 보고되고 있다. 특히 최근 골다공증이 있는 고령인구의 증가와 더불어 쇄골 간부 골절과 같은 금속판 근위부의 합병증도증가하는 추세이다. 본 논문의 2가지 증례에서는 갈고리 금속판을 이용한 수술 이후 금속판 근위부의 쇄골 간부 골절이 발생하였으며, 보존적 치료를 통해 골유합과 좋은 임상적 결과를 얻을 수 있었다. 이에 제5형 원위 쇄골 골절에서 갈고리 금속판을 이용한수술적 치료 이후 발생한 쇄골 간부의 금속판 주위 골절 2예에 대해 문헌고찰과 더불어 보고하는 바이다 A hook plate is commonly used to treat distal clavicle fractures or acromioclavicular joint dislocations with good clinical results. On the other hand, several complications related to the structural characteristics of the plate or the surgical procedure can occur. Several studies have reported problems with the distal part of the hook plate, such as acromion fracture or rotator cuff tear. Proximal problems, such as clavicle shaft fractures, are increasing owing to the increase in the elderly population with osteoporosis. In these two cases, after surgery using a hook plate, clavicle shaft fractures occurred proximal to the plate. Bone union and good clinical results were obtained through conservative treatment. This paper reports two cases of periprosthetic clavicle shaft fractures after surgery for type V distal clavicle fractures using a hook plate with a review of the relevant literatures.
